CONTENT PROPERTY INSURANCE FORM:

Property & Receiving Department is working on to keep current on the content for programs to be successful we need your assistance to complete the Property Insurance Content Listing.  Below is the form to be used, but if you contact property@standingrock.org you can retrieve an Excel spreadsheet.  The content needs to compiled by offices/cubical, Computers, conferences, etc.

HOW TO FILE A VEHICLE CLAIM (PDF or PRINT)

To file a claim, please submitted the following:

1.)           Please adhere to the Standing Rock Sioux Tribe Vehicle & Fleet Guidelines: ACCIDENTS/INCIDENTS
a. All accidents in Tribal vehicles whether owned, leased, rented or borrowed must be reported Immediately to the Tribal Police and the Property & Receiving Office, giving details of the accident, to include but not limited to the following information:

a1. The name, address, and phone numbers of those involved and of any witnesses.

b1. All pertinent information the other vehicle(s) if any.

Note: At no time shall the employee admit fault until the Tribes review the circumstances. In case of injury or death, notify the us immediately (701) 805 8107 / property@standingrock.org

  1. When an accident occurs the driver should follow the instruction on the accident kit, found in the glove compartment of all vehicles. each vehicle should have the proper forms in the glove compartment on reporting and accident. If none is in their please print “Claim Form” along with “Insurance Card” and place.

a1. All accidents must be called in immediately to Program Manager/Director or other designated individual and to Property & Receiving Department.

b1. If an employee is responsible for the accident/incident, they will be immediately accompanied by their Supervisor to complete a post-accident UA, (if the employee tests positive for drugs or alcohol, then they will be subject to disciplinary action as outlined in the Tribes. Employee Policy Manual, (EPM).

c1. A claim must be filed for all accidents, whether the driver’s fault or not.

  1. Obtain a Police Report or Report to submit to Property & Receiving Department Via email at property@standingrock.org if GSA that form will need to be filled out as well and sent in.
  2. Notification to Property & receiving
  3. Complete Claim Form 2024 and submit to Property & Casualty via email to property@standingrock.org
  4. According to the Employee Policy Manual (EPM), post-accident testing needs completed. Please see EPM or visit with your director/Human Resources Department.
  5. Three (3) estimates from a vendor for repairs must be established and attached as well.
  6. Pictures, we need pictures of the vehicle, Pics are required picture is a picture of the front or back of the vehicle to include a license plate. Must be attached to finished paper work in full color.

Note:  Deductible is $500.00 and will need to be paid directly to vendor once claim is approved for repairs.

SRST ARTIFACTS

The SRST artifacts are privilege and honor for us to be caretaker of these items, we need to have them placed on our inventory in case an unfortunate incident.  Tribal artifacts is property of the Standing Rock Sioux Tribe which we have purchase or given to us.  The Property & Receiving Department would require pictures must accompany the Tribal Artifacts by Program Sheet (See Below).  You may obtain an Excel Spreadsheet by contact property@standingrock.org

SRST Inland (Equipment) Marine inventory is vital to keep current, please use the attached form to add/delete equipment that values at $5,000 or above.

NOTE: ATV’s / SIDE BY SIDES can obtain a deductible of $500 IF certified Road Worthy Certificate Documents must be presented:

If your program has lost, had an item stolen, or misused/abused, no longer needed, not economical to repair, trade in, auction, dispose for scraps, retain for parts, turn into Recycling or Transfer to another program than please use the following form:
